Abstract:
The article evaluates the compatibility of the current National Health Service of Ukraine (NHSU) contract model with the regulatory and architectural logic of Regulation (EU) 2025/327 on the European Health Data Space (EHDS) under martial law. The study aims to determine the implementation level of the feedback loop between electronic health records quality and financial decisions within the Medical Guarantee Program (MGP), alongside identifying systemic gaps on the path toward European integration of the digital healthcare system. The methodological framework is built on a systematic-structural analysis of the regulatory framework, statistical processing of open data from the NHSU automatic monitoring regarding 857 healthcare providers for 1–10 months of 2025, and a qualitative generalization of 52 monitoring visits. The results prove that the NHSU operational tools (payments based on verified data, corrective coefficient 0, risk-oriented control) are structurally adjacent to the logic of primary data use in EHDS. At the same time, critical systemic gaps have been identified: a large-scale semantic fragmentation of medical information systems (MIS), which led to unjustified financial recalculations amounting to over 3.15 billion UAH due to incorrect diagnosis-related groups (DRG) determination, and a methodological incompatibility of primary care data time series after introducing mandatory coding rules in August 2025. The scientific novelty lies in integrating the EHDS analytical framework into the MGP financial tools evaluation, which allows classifying semantic coding errors as a direct managerial and financial risk. The practical value consists in substantiating a three-level conceptual model (standardization – verification – secondary use) that closes the feedback loop between health data quality and tariff-setting through the establishment of a Health Data Access Body (HDAB) and a terminology FHIR-server, minimizing financial risks for providers and enhancing budget allocation transparency.
